By submitting this application, you are agreeing that all information you provided is accurate and true. You also are agreeing to all the terms of service laid out in the Term of Service statement. You are agreeing that if you are accepted as a Blade Radio staff member, you shall remain professional on all forms of social media; up to an including, but not limited to: Not bad mouthing other stations, members of the gaming community or staff from other stations, cultures or gaming communities/companies. You also acknowledge that we do reserve the right to refuse any application at, any time, for any reason, as we see fit.


In consideration for becoming a Blade Radio staff member, on behalf of myself and my next of kin, heirs and representatives, I release from all liability and promise to not sue Blade Radio, their employees, officers, directors, volunteers, agents, affiliates, sponsors and owners (Here in known as the "Company") from any and all claims, including claims of negligence, resulting in any physical or psychological injury (including paralysis and death), illness, damages, or economic or emotional loss I may suffer because of my participation in the Company, including travel to, from and during my time working for the Company. I assume all related risks, both known and unknown to me, of my participation in the Company, including travel to, from and during the time I work for the Company. I agree to hold the Company harmless from any and all claims, including attorney's fees or damage to my personal property, that may occur as a result of my working for the Company, including travel to, from and during the time I work at the Company. If I need any form of medical help or treatment, because of my participation or lack there of, I agree to be financially responsible for any costs incurred as a result of such treatment. I further more agree that I am at least 18 years of age or older. If I am only 16 years old, I agree that I must have my parents sign a consent form and that by them doing so, they agree to everything you have agreed to in regards to applying for a position with the Company.
